A Frontline Battle: The Mental Health Crisis in Emergency Medical Services

Introduction More than half a century has elapsed since the first International Congress of Mental Health was constructed in 1948. 1 Since then, great progress has been made in the field of mental health education, resulting in a greater acceptance and understanding of mental illnesses and their potential consequences. Despite these advancements, the emergency medical services (EMS) community has remained among some of the highest rates of occurrence of mental illness paired with some of the lowest likelihood of seeking treatment. 2 This text is designed to review the occurrence of mental health issues among EMS personnel, factors that contribute to this phenomenon, and suggestions for how to better manage EMS providers’ mental well-being. It is no secret that an occupation within EMS results in a high-stress work environment. In addition to general job-related stressors; poor sleep schedules, and repeated exposure to hazards and tragedy all contribute to the association between EMS personnel and the occurrence mental illness. Download Full Report The American Ambulance Association (AAA) and Newton 360, an Emergency Medical Services (EMS)-based workforce management software platform, joined forces again this year to conduct the fourth annual survey of employee turnover in the EMS industry. The 2022 survey presents turnover data from 119 EMS organizations, representing more than 12,000 employees. This year, the survey included data regarding the percentage of positions reported to be open at the time of the survey, which showed that EMS agencies are having difficulty filling open positions. Over the last few years, EMS organizations were impressive in meeting the challenges in the face of the continuing COVID-19 global pandemic. The EMS workforce rose to the challenge and battled the disease, despite the workforce shortages that have plagued the EMS profession and other healthcare partners for nearly a decade. EMS organizations have offered innovative solutions to public health crises, including establishing testing and vaccinations sites around the country, suggesting that EMS employers and employees are prepared to expand their role to include greater provision of preventive services and community-based healthcare. Editor's Note: On August 1st, 2022, EMS1, Fitch & Associates, and the National EMS Management Association released their fifth annual senior partner at Fitch & Associates. — Translating data into information, and information into action Survey questions for the EMS Trend Survey were developed through a collaboration between EMS1, Fitch & Associates and the EMS Survey Team – which was instrumental in collection and interpretation of data. The EMS Trend Survey was conducted online in early 2022. The survey was widely promoted and open to any member of the EMS community. More than 3,200 people responded, the most in the survey’s history. They represent a wide range of service models, certification levels and years of experience, and every region of the nation. The 2022 EMS Trend Survey results reinforce what many of us see every day. More than two years into the pandemic, it’s still impacting every aspect of our profession – from recruitment and retention, to safety, to how you assess and treat patients. As we have realized that this pandemic is not just a flash in the pan, but something we may be dealing with for a long time, we have also been able to move forward.
