Passports Thousands of Post Offices ™ accept first-time passport applications for the U.S. Department of State. Most of those locations can also take your passport photo. • Renewing Passports: If you answer 'Yes' to all 5 eligibility questions in the " • First-Time Passports (and Passport Photos): Schedule your in-person appointment online. • Complete your • Print your completed application. There are two types of passport offices in the United States: Passport Acceptance Facilities and Regional Passport Agencies. Passport Acceptance Facilities are local passport offices that can provide passport applications, review passport applications, and verify your identity and US citizenship as a preliminary step to getting your passport. When you are applying for a first time passport, a passport for a child, or a replacement for a lost or damaged passport, the Passport Acceptance Facility will seal your application into an official envelope in a process called “executing the application”. Types of Passport Offices There are two types of passport offices in the United States: A Regional Passport Office (also called a Regional Passport Agency) is often located in a major city and is available by appointment only. Each facility has it’s own process and scheduling so you may need to plan in advance if you wish to visit a Regional Passport Office. Expect to wait in line if you need to get your passport at your local office. The benefit of a Regional Passport Office is the possibility to get your passport in an emergency rush. • One passport photo (photo services available at the library for $15). • Original birth certificate (with raised seal) OR naturalization certificate. Old passport can be used as proof of citizenship for adult renewals. Social security number must be printed on application. • Valid driver’s license (copy of front and back). • Out-of-state driver’s license (other than CT or NY) and license issued less than a year prior must submit a second form of ID. Social security form accepted as secondary ID. • Personal or bank checks (no cash accepted) for U.S. Dept of State payment. • Proof of name change (marriage certificate, court order for name change or divorce decree). • Ages 16 – 18: bring your own ID, or bring one parent/guardian to identify you. • Original birth certificate with raised seal OR naturalization certificate for first time applicants. Copy of birth certificate and old passport for renewals. • Both parents/legal guardian must be present or submit a NOTARIZED written consent statement by non-appearing parent with a copy of his or her driver’s license, front and back for both first time and renewals.
