Total Thyroxine (TT4) and Total Triiodothyronine (TT3) Methods

Introduction Thyroxine (T4) is the principal hormone secreted by the thyroid gland. All the T4 in the circulation is derived from thyroidal secretion. In contrast, only about 20% of circulating triiodothyronine (T3) is of thyroidal origin. Most of the T3 in blood is produced enzymatically in nonthyroidal tissues by 5'-monodeiodination of T4. [ [ [ Technically, it has been easier to develop methods to measure the total (free 1 protein-bound) thyroid hormone concentrations, as compared with tests that estimate the minute free hormone concentrations. This is because total hormone concentrations (TT4 and TT3) are measured at nanomolar levels whereas free hormone concentrations (FT4 and FT3) are measured in the picomole range and to be valid, must be free from interference by the much higher total hormone concentrations.

A complete pathway map of T4 and T3 metabolism and clearance – Thyroid Patients Canada

Biochemical pathway maps are an important aspect of medical education and medical problem-solving. They help us to view a network or series of chemical reactions in cells. In this article, I present a comprehensive science-based pathway map showing all the major pathways of both T4 and T3 hormone metabolism, also including their urinary clearance. I also offer a secondary pathway map zooming in on T3 metabolism and clearance alone, which is meant to be an appendix or companion to the more T4-centric map featuring both hormones. If you find this article too complex and scientific for your taste, I recommend my previous post, which offered a visual analogy of these pathways for beginners and laypeople —“ The pathway maps I display here are based on a thorough review of the science. Fragments of the full pathway map have been portrayed in many thyroid science publications since the 1970s, and I show some of those fragments at the end of the article. However, it is currently difficult to find a complete metabolic map that includes all the major pathways of T4 and T3 at a glance. A complete visual showing of both hormones’ metabolic and renal losses is an essential tool for troubleshooting challenging cases — especially during thyroid therapy or illnesses when several health conditions and treatments create metabolic imbalances and make lab results unusual. Most visual models lack various parts and fail to show T3 pathways alongside T4 pathways. Most portrayals of T3’s pathways...

T4 Blood Test: What Is it Used For?

T4 is short for thyroxine, which is one of the hormones made by your hormone gland. A T4 test shows how well your thyroid is working to regulate your body’s growth and energy. About the T4 Blood Test Your body makes two different T4 hormones. The first is a free T4 that flows through your bloodstream independently. Bound T4 is the second type of thyroxine. It travels through your bloodstream by attaching to proteins. Your doctor may order a total T4 test to measure free T4 and bound T4, or they may order a free T4 test that doesn’t include results for bound T4. Many doctors order the free T4 test because of accuracy concerns surrounding the bound T4 results. ‌‌ While the primary reason for a T4 test is to check your thyroid, it can also alert your doctor to Procedures for the T4 blood test. Talk to your doctor about any medication, vitamins, or supplements you’re taking. Medications for seizures, heart conditions, and birth control may affect the results of the test. If you take a biotin supplement, it may cause a false-positive test result, showing that you don’t have a thyroid problem. Some infections may also affect your test results. This includes chronic diseases like kidney failure or cirrhosis. Cirrhosis is scarring on your liver which can be caused by different diseases and conditions, including hepatitis and alcoholism. If you have a short-term infection, your doctor may want to wait to do the test until you feel better. Reason for a T4 Blood Test There are a few ...
