We all like an underdog story. Many news stories portray rural healthcare as the underdog as many organizations delivering rural healthcare fight for survival. Yet the story that is being written about rural healthcare is missing a key element. A new narrative is needed to better capture the opportunities that underpin rural healthcare. While much attention has been focused on rural healthcare challenges and disparities, not as much focus has been given to the possibilities for innovation and the role of emerging leaders. Cheering for the underdog is much easier when you know that there is great hope for them to win. Current Literature — The Good, The Bad, and the Ugly So where does some of this hope originate? Rural Health Grants There are numerous rural health grant opportunities aimed at improving the quality of health care in rural areas—made available to hospitals in the form of public grants, federally-funded demonstration projects, and private grants. The single best resource for federal funding of rural health grant programs is Grants.gov. Use the “Search Grants” drop down menu to identify rural health grant opportunities by agency (e.g. HRSA or CMS). Current federal grant opportunities include: HHS’ Grants by Location – US by Metropolitan and Non-Metropolitan report provides grant data sorted by the Fiscal Year, Organization, State, and County Classification. Grant award distribution can be further refined by selection of Metropolitan, Non-Metropolitan, or Unknown grouping parameters, using the definitions provided by OMB's county-wide designations. Rural Healthcare Foundation operates a chain of primary health care units that offer high quality, affordable, outpatient services in low-income, densely populated regions where health care delivery is sparse. Services include general medicine, opthamology, dental and homeopathy. This organization has developed earned income revenue model to ensure clinic chain is self-sustaining. •

People living in rural areas have many challenges to accessing quality, affordable health care. But the situation is not hopeless. There are patients, advocates and providers who are developing innovative, effective approaches to improving health care in their communities. We are here to shine a light on these projects and the passion that drives them. We recognize that “rural” encompasses a wide diversity of people who share many of the same problems but also bring distinct cultural perspectives to their health care experiences. We welcome your comments and contributions to this effort. Promises to Keep Freddie White-Johnson grew up in rural Mississippi and still lives there. At age 17, she promised her father who was dying of cancer that she would get an education and “help the people,” and she has done that all her life. She founded the Fannie Lou Hamer Foundation to raise awareness of cancer and is working to fund and build a much-needed cancer center for her community. A Messenger for Hope Alma McCormick is a member of the Crow Nation, living in Montana. Years ago, after losing her infant daughter to a rare cancer, she vowed to do what she could to make sure other people didn’t suffer as she had. She works with Crow people to raise awareness of cancer and improve access to health care.
