Sodium Nitroprusside Injection is not suitable for direct injection. The solution must be further diluted in sterile 5% dextrose injection before infusion. Sodium Nitroprusside can cause precipitous decreases in blood pressure (see ). In patients not properly monitored, these decreases can lead to irreversible ischemic injuries or death. Sodium nitroprusside should be used only when available equipment and personnel allow blood pressure to be continuously monitored. Except when used briefly or at low (< 2 mcg /kg /min) infusion rates, sodium nitroprusside gives rise to important quantities of cyanide ion, which can reach toxic, potentially lethal levels (see ). The usual dose rate is 0.5-10 mcg /kg /min, but infusion at the maximum dose rate should never last more than 10 minutes. If blood pressure has not been adequately controlled after 10 minutes of infusion at the maximum rate, administration of sodium nitroprusside should be terminated immediately. Although acid-base balance and venous oxygen concentration should be monitored and may indicate cyanide toxicity, these laboratory tests provide imperfect guidance. What Is Nipride RTU? Nipride RTU (sodium nitroprusside) in 0.9% sodium chloride injection is a direct acting vasodilator indicated for immediate reduction of blood pressure; producing controlled What Are Side Effects of Nipride RTU? Common side effects of Nipride RTU can be serious and include: • • • thiocyanate toxicity, • blood disorder ( • increases in intracranial pressure. Less common side effects of Nipride RTU include: • slow or fast heart rate, • rash, • nausea, • abdominal pain, • muscle • dizziness, • headache, • flushing, • sweating, • red streaking along the veins, and • irritation at the infusion site. Dosage for Nipride RTU Initiate infusion of Nipride RTU at a rate of 0.3 mcg/kg/min, and titrate every few minutes until the desired effect is achieved OR the maximum recommended infusion rate of 10 mcg/kg/min has been reached. What Is Nitroprusside Test? Nitroprusside test is a biochemical test used for the detection of the free-SH groups in amino acids or the cysteine amino acid in a protein. This test uses sodium Nitroprusside reagents and ammonia solution. Nitroprusside is a complex anion consisting of an octahedral ferrous center which is surrounded by five tightly bound cyanide groups. In this test, the thiol groups (cysteine) react with sodium Nitroprusside in the presence of excess ammonia to give a red or purple color. Nitroprusside test is commonly used to detect the presence of ketones in the urine. The test can also be used for the differentiation between cysteine and cystine as cysteine gives a positive result to this test but cystine does not.
